Online Preliminary Enrollment Application
Note: Before submitting the preliminary enrollment application, please be sure to carefully read the New Creations Basketball Academy Handbook. The initial questionnaire must also be submitted first with approval received from the New Creations Basketball Academy staff before submission of the application.
Be sure to fill out the application completely, giving explanations where needed. If you prefer to print and mail the preliminary enrollment application form instead, you can download a pdf version. If additional space is needed, use the "comments" area at bottom.
New Creations Christian School, Boarding School, Basketball Academy, and Bible College admits students of any race, color, and national or ethnic origin. This school is authorized under Federal law to enroll non-immigrant students. New Creations Basketball Academy accepts both male and female students.
